Thank you for choosing Midwest Animal Rescue & Services (MARS) to assist with  re-homing  your pet.

*All  adopted & returning  MARS dogs need use this form also. We follow the same process as a non-MARS adopted pet.

Please note: Once the form is received, an intake assessment will be scheduled to ensure your pet is eligible to be brought into our adoption program. You will be contacted within 72 hours to schedule the intake assessment. We ask that at the appointment you bring all medical records showing current shots and spay/neutered, as well as any other paperwork on your pet, favorite toys, blanket/bed and food.  Please review the process here:   http://www.midwestanimalrescue.org/info/display?PageID=14388

Please also note that you will most likely be taking your pet home after the intake assessment. It usually takes approximately a week for us to match a foster with the pet.  Please allow enough time for us to find a foster.  We do not have the ability to take your pet on the spot. Extenuating circumstances will be handled on a case by case basis.

There is no surrender fee, but we do ask for a donation.  No matter how small, it helps to cover the costs of re-homing your pet.  Please note that we will consider every dog/cat with or without a donation.
